There is no pressure to share everything at once. Your provider will move at a pace that feels comfortable for you.
What to Bring
To help your appointment go smoothly, please bring:
• A photo ID
• Insurance information if applicable
• Completed intake paperwork if requested
• Any questions you would like to discuss
If you forget something, our team will help guide you through the process.
After Your Appointment
At the end of your visit, your provider may recommend next steps based on your goals and needs.
